The following information should be included in your request for a grant:

1. A copy of your tax exempt letter issued by the internal revenue service. It will state what type of tax organization you are. Do not confuse this with your employer identification number which is used for payroll preparation.
2. The proper name and address of your organization. Please indicate if you have moved or are a local office of a national non profit organization.
3. A description of what the grant will be used for and whether or not this is to be a matching grant.
4. The amount of the request.
5. Grants now need a set of financial statements with the request.
